Program Information:
The purpose of this seminar is to provide the ACCME accredited programs that address the clinical practice needs of osteopathic physicians. Sessions are offered to continually assess issues, developments, advances and deficiencies in current medical practice, and address identified needs through relevant comprehensive programs designed for general statewide physician audiences of all specialties.

CME Credits:
CME Verification will be completed online. Credit will be given only to physicians who complete the online attestation form indicating the total number of hours attended. No credits will be given to those physicians who fail to complete their online CME Verification Form.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the American College of Osteopathic Family Physicians (ACOFP) and CAOM. ACOFP is accredited by the ACCME to provide continuing medical education for physicians.
ACOFP designates this live activity for a maximum of 24.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
CME Verification:
Simple! Participants must login to the virtual conference and complete the online CME Verification form for presentations you participated. Credit will be given only to physicians who complete the online attestation form indicating the total number of hours attended. No credits will be given to those physicians who fail to complete their online CME Verification Form. Credit will be submitted directly to the AOA by the American College of Osteopathic Family Physicians. A certificate will be provided for attendees requiring AMA PRA Credit.
Cancellations, Refunds, and Grievances:
Refunds for cancellation will be granted in full only if notification is received two weeks before the event (January 9). After this date, refunds are no longer available. This program is designed to comply with Uniform Guidelines for Accrediting Agencies of Continuing Medical Education. All grievances will be responded to in writing within 30 days of receipt. Initially, all grievances should be directed in writing to the CAOM Executive CME Committee, PO Box 1989, Elyria, Ohio 44036.
